Code "V" Electronically In 2008!

If you have provided us with an email address in the past, or provide one with your renewed or new membership, we will begin sending your Code "V" electronically at a date in the near future.

It will arrive in your inbox as a PDF file. If you do not have Adobe Reader on your computer, you can download this program for free at http://www.adobe.com/products/acrobat/readstep2.html If you do not wish to receive your copy of Code "V" electronically you may decline this option (on this form) or by notifying the CVSA office, and you will continue to receive a copy by mail.
